About this role

The Social Prescribing Community Referral Link Worker will collaborate with the Project Co-ordinator and the National Forum of Family Resource Centres. This role focuses on developing and delivering a Health & Wellbeing Community Referral initiative that connects individuals to community-based supports, addressing their social, emotional, and practical needs.

You will support individuals to improve their health and wellbeing through a person-centred service. Your work will involve assessing needs and co-producing plans, ensuring a strong commitment to non-judgemental practice and respect for diversity.

• Relevant third-level qualification in Social, Community, Health, or a related field
• Minimum of 3 years’ experience in community development, healthcare, or related area
• Proven ability to support individuals on a one-to-one basis
• Skilled in motivational interviewing and active listening
• Confident working with individuals facing complex needs
• Strong commitment to non-judgemental, person-centred practice
• Knowledge of health inequalities and experience in reducing them
• Excellent communication, interpersonal, analytical, and problem-solving skills
• Strong IT skills, including social media and report writing
• Experience in data collection, monitoring, evaluation, and service development
• Understanding of safeguarding and confidentiality
• Full, clean driving licence and access to a car

• Salary of €50,407.37 per annum
